[MakeLV] CNC/Laser cutting offer

Here's a thing I designed which I kind of want to get made:

Right now it's thick and chunky, if you can make it for me I would probably
thin it down first. If you think you can get it to me before christmas
maybe I'd write my wife's name on it. Any guess how much to get it made out
of plastic? Any design tips?
